In order to generate a referral, please fax (888-501-1009) or email (info@ImproveAbility.com) the following info:
- Client contact info
- Disability info – good intake note, neuro-psychological evaluation, or other documentation of disability
- Completed TWC Referral Form – PLEASE MAKE SURE TO TELL US WHAT THE VOCATIONAL GOAL IS
- Service Authorization (SA) for Assistive Technology Evaluation for:
- As of 9/1/2024 $1140.00 – flat rate for Assistive Technology Evaluation – this can be entered into TWC system as 24 units @ $47.50/unit
- This flat rate covers all admin time to schedule and confirm appointments, time meeting with client to determine AT needs, research to find AT solutions, report writing time, and admin work to submit our comprehensive report and invoice for payment. We do not charge based on time.
- There are occasions where extensive time is spent attempting to contact clients, but we are unable to complete our visit. In these cases, we will charge $380.00 for admin time spent attempting to schedule. Please note that we will exhaust all efforts including circling back with TWC to get additional contact info, but if we are unable to schedule with a client due to no response from them or not having correct contact info, or the client not attending a scheduled and confirmed appointment, we will invoice $380.00 for the work done.

- We do not charge travel for clients that live/work within 30 miles of our office address:

- We always try to combine trips. We ask that the PO include full round trip travel, but we will only charge you for travel used for your client.

- We may perform the evaluation in person or via remote video visit. We will work with the client to determine the most appropriate setting for our evaluation.

PLEASE NOTE: After scheduling with clients, we confirm all appointments within a few business days of the appointment. We will not go to a client’s location if the appointment is not confirmed. If a client does not show for a remote appointment, or if they are not able to otherwise meet with us on the scheduled and confirmed date and time, we will charge TWC for administrative time used. We may ask for a new referral if the appointment cannot be rescheduled quickly.
We do our best to see clients as quickly as possible, but we ask that you give us 30 days to schedule with clients. We are typically able to schedule with clients within 2-3 weeks of receipt of referral, but occasionally schedules require us to schedule further out.
Our current turn around for reports is 3 weeks (or less) from the date of the evaluation.
